I think that when screening patients, there should be a question as to their language preference. While waiting for my hearing test, with others in the room, the technician came out; and in a kindergarten teacher voice, hesitantly asked if I needed a Spanish interpreter. Now, I was a bit taken aback, as I did not express the need for one when I made the appointment. However, in hindsight, it would be helpful to her if that need is identified (and annotated) whilst making the appointment initially. Then neither the patient, nor the technician needs to feel awkward.
